Hospice Registered Nurse; PRN

Role Overview: As a Per Diem Registered Nurse Case Manager, you will provide comprehensive nursing care for hospice patients, ensuring the delivery of personalized plans of care. You will evaluate patient needs, initiate and coordinate care plans, document patient progress, and collaborate with patients, families, physicians, and interdisciplinary team members to promote quality care.

• Conduct initial nursing evaluations within 48 hours of referral to determine hospice eligibility.
• Assess and reassess patient and family needs at least every 15 days.
• Develop, implement, and update care plans while maintaining accurate clinical documentation.
• Identify problems, set appropriate goals, and evaluate patient and family responses to care.
• Work collaboratively with the patient, family, and healthcare team to provide consistent and compassionate support.

• Current unrestricted RN license in the practice state.
• At least 1 year of clinical nursing experience.
• Valid driver's license, reliable transportation, or access to public transit.
• Preferred: CPR certification or ability to obtain within 90 days.
