** SUMMARY The Population Health Nurse supports the health and wellness of the patient population through preventive care initiatives, immunization program management, quality improvement activities, and data-informed outreach. This position serves as the clinical lead for vaccine management and the Vaccines for Children (VFC) program while supporting organizational quality and population health goals.

DUTIES AND RESPONSIBILITIES include, but are not limited to the following:

Use electronic health record (EHS) tools and reports to identify care gaps and support preventive service outreach (e.g., immunizations, cancer screenings, well-child visits, annual exams, and advanced directives).Implement standing orders and evidence-based protocols to improve preventive care rates.

Support chronic disease registries and targeted health promotion initiatives.

Conduct limited outreach to patients for high-priority preventive services.

Track and support performance on quality measures.

Participate in quality improvement projects related to preventive care, immunizations, and screening rates.

Plan and implement population-based health initiatives

Collaborate with internal programs and community partners to promote wellness and disease prevention, health equity.

Address barriers to care including transportation, financial concerns, health, literacy, and social determinants of health through referral and collaboration with community and internal partners

Collaborate with specialists and community partners to ensure well coordinated care Provide patient education on disease self management, medication, adherence, and lifestyle improvements

Monitor and track high needs patients and ensure care needs are met through individualized care plans

Serve as the clinical lead for vaccine storage, handling, and administration practices.

Manage and maintain compliance with the Vaccines for Children (VFC) program

Monitor vaccine inventory, ordering, reconciliation, and reporting.

Ensure proper vaccine cold chain management, temperature monitoring, and documentation

Coordinate vaccine-related audits and site visits

Provide staff training and competency validation on vaccine protocols

Monitor immunization rates and implement strategies to improve coverage across the lifespan.

Maintain accurate and timely documentation in the EHR and state immunization registry.

Develop and monitor dashboards related to immunizations and preventive health metrics

Support reporting requirements for grants

Provide culturally responsive health education to patients, families, and the community

Support response efforts for communicable disease prevention and control as needed

Work collaboratively within the integrated care team Provide clinical and operational support including assisting with nurse triage, care coordination, and clinic operations/patient flow as needed to support efficient care delivery.

Conduct home or community visits as needed to support community health team and perform community outreach.

Provides day-to-day supervision of clinical staff, including work assignments, time-off requests, scheduling, and payroll administration.

Partners with the Nurse Manager to address employee performance concerns and disciplinary matters.

Performs other duties as assigned. These may include but are not limited to:

Maintaining professional and technical knowledge by attending workshops, conferences, as well as, self-directed learning and continuing education opportunities
