Licensed Practical Nurse (LPN) | Providing Compassionate Care and Meaningful Connections Wellspring Skilled Nursing & Rehab – Fairview | Full-time, Part-time or Per Diem | Hourly $29.50 - 32.40

As an LPN, you'll be a vital member of the care team, helping residents maintain their health, comfort, and well-being while creating an environment where they feel respected, supported, and t importantly, you'll help residents experience the dignity, comfort, and quality of life they deserve.

What Your Impact Will Look Like You'll provide hands-on nursing care that supports residents physically, emotionally, and socially.

You'll spend time getting to know residents, understanding their unique needs, and helping ensure their care reflects their personal preferences and goals.

You'll serve as an advocate for residents while collaborating with physicians, nursing assistants, therapists, and other members of the interdisciplinary team to provide coordinated, person-centered care.

You'll monitor changes in condition, communicate concerns, implement physician orders, and help ensure residents receive timely interventions that support positive outcomes.

You'll help guide and support nursing assistants during your shift while fostering teamwork and a shared commitment to resident care.

You'll contribute to admissions, discharges, care planning, and ongoing assessments while helping create a safe and welcoming environment for residents and families.

Some days you'll be providing comfort to a resident who needs reassurance.

Other days you'll be supporting a family through important healthcare decisions, coordinating care with physicians, or identifying a subtle change in condition that prevents a larger health issue.

What You'll Bring Current Michigan Licensed Practical Nurse (LPN) license.

Graduation from an accredited Practical Nursing program.

Current CPR certification or willingness to obtain CPR certification.

Nursing experience in skilled nursing, home health, medical-surgical, critical care, or related settings preferred.

Strong clinical, assessment, communication, and documentation skills.
