LVN​/LPN – LTC​/SNF

Job Summary:

Provides nursing care to patients in a long-term care setting, ensuring safety and comfort in accordance with established policies and procedures. Supports staff nurses and demonstrates a commitment to patient-centered care.

Responsibilities:

• Assist staff nurses in the treatment and daily care of patients.
• Adhere to infection control protocols and facility policies, including attendance and punctuality standards.
• Document patient information accurately and in a timely manner.
• Demonstrate understanding of patient and family-centered care principles.
• Follow established clinical methods and procedures to support optimal patient outcomes.
• Maintain professional appearance and compliance with dress code standards.

Qualifications:

• Minimum of 2 years of Long-Term Care (LTC) experience.
• Active BLS certification from AHA.
• Valid Michigan LVN/LPN license.
• Prior experience with MI LTC Fingerprinting is preferred.
• Ability to communicate effectively with patients, families, and staff.
• Reliable attendance record with a strong commitment to punctuality.
