Licensed Vocational Nurse​/LPN-Long Term Care

LPN – SNF/LTC JOB SUMMARY:

The LPN provides nursing care to residents/patients under the supervision of the Unit Manager/ADON/DON. Responsible for assessing residents, implementing and evaluating care plans, maintaining a safe environment, and supervising nursing assistants and support staff.

• Provide nursing care in accordance with established standards and the State Nurse Practice Act.
• Assess residents and implement/evaluate individualized plans of care.
• Monitor resident safety and maintain a supportive care environment.
• Supervise nursing assistants and helpers in resident care activities.
• Follow facility policies, procedures, and applicable quality standards.
• Provide excellent customer service and compassionate resident care.
• Demonstrate professionalism and teamwork throughout the assignment.

• 2+ years of SNF/LTC experience – Required.
• Active Michigan LPN license – Required.
• BLS through AHA or American Red Cross – Required.
• COVID-19 vaccination – Required
  ; medical/religious exemptions may be considered case‑by‑case.
• Previous travel experience – Required.
• Flexible scheduling required —
