Case Manager - Superior Futures for Youth (Michigan)

Lutheran Social Services of Wisconsin and Upper Michigan (LSS) is seeking a full-time Case Manager to join our growing Superior Futures for Youth program. This program serves youth experiencing homelessness across Delta, Menominee, and Schoolcraft Counties, with this position primarily supporting Dickinson and Iron Counties
.

This role provides housing stabilization and supportive case management services to youth ages 12-24
, helping them achieve long‑term stability and independence. The ideal candidate is passionate about youth services, highly organized, and comfortable working independently in community settings.
Travel up to 75% throughout the service area is required. The ideal candidate would reside within one of the counties served.

This is a full‑time (40 hours/week), benefits‑eligible position
.

Position Summary

Under the direction of program leadership, the Case Manager delivers professional‑level social work and case management services to individuals and families. This role involves assessing client needs, coordinating services, and providing ongoing support to help youth achieve stable housing and improved well‑being.

Key Responsibilities
  • Conduct comprehensive assessments through interviews, collateral contacts, and other evaluation methods
  • Develop individualized case plans in partnership with youth and ensure delivery of agreed‑upon services
  • Provide ongoing case management and coordinate housing, social, and mental health services
  • Build and maintain collaborative relationships with community partners and service providers
  • Maintain accurate and timely case records, documentation, and required reports
  • Participate in court proceedings or hearings as required
  • Promote program services and accept referrals from community sources
  • Participate in staff development, trainings, team meetings, and professional learning opportunities
  • Support quality improvement efforts and contribute to a positive team environment
  • Represent LSS in the community, including outreach and engagement activities
  • Perform additional duties as assigned
Qualifications

Required:

  • Bachelor's degree in Social Work, Sociology, Psychology, or a closely related field (or equivalent combination of education and experience)
  • Minimum of two (2) years of professional experience in human services or related field
  • Knowledge of case management practices, social service systems, and community resources
  • Understanding of social, behavioral, and mental health challenges affecting youth
  • Strong organizational, communication, and documentation skills
  • Ability to work independently and manage time effectively
  • Valid driver's license, reliable transportation, and ability to meet LSS driver requirements

Preferred:

  • State certification such as Certified Social Worker (CSW),
    Certified Professional Counselor (CPC), or equivalent
Travel Requirements
  • Travel within service counties up to 75% of the time
  • Occasional overnight travel may be required
