Horizon Case Management provides collaborative case management and care coordination services for individuals recovering from auto accidents, workplace injuries, and complex medical conditions. Guided by the CMSA standards, we focus on advocacy, communication, and coordination to ensure every client receives safe, effective, and compassionate care.

Role OverviewWe are seeking a compassionate, detail-oriented Field Case Manager to support patients and collaborate with a multidisciplinary clinical team. This role involves direct patient interaction, care coordination, advocacy, and field-based travel.
Key Responsibilities- Coordinate and manage patient care plans in collaboration with the clinical team
- Advocate for patients and monitor health status, reporting new or changing conditions
- Maintain accurate, timely documentation and case notes
- Communicate effectively with patients, families, providers, and internal stakeholders
- Manage multiple cases while maintaining organization and attention to detail
- Travel to client locations (up to 80 miles as needed)
- Work effectively under pressure in dynamic, real-world environments
- Bachelor’s degree (any field) OR:
- Licensed Practical Nurse (LPN) or Registered Nurse (RN) accepted in lieu of degree
- Strong communication, organization, and teamwork skills
- Demonstrated integrity, accountability, and reliability
- Compassionate and patient-focused mindset with strong advocacy skills
- Ability to manage responsibilities in a fast-paced, high-pressure environment
- Valid driver’s license, reliable vehicle, and active auto insurance
- Willingness and ability to travel regularly for patient visits (up to ~80 miles)
- Authorized to work in the U.S.
- Degree in healthcare, social services, or related field
- 1–2+ years of experience in healthcare or case management
- Experience working with:
- Traumatic Brain Injury (TBI)
- Spinal cord injury patients
- Case management or patient-facing experience (hospital, clinic, home care, or similar)
- Bilingual or multilingual abilities

- Employees are expected to pursue professional certifications such as:
- Certified Brain Injury Specialist (CBIS)
- Eligibility and preparation may begin after the first year of employment
- Company may cover certification costs once (conditions apply)
