Location:
Ehardt/Children’s/Administrative/Croswell Site, Michigan.
- Gather and review medical, psychological, and social history to assess the individual's needs and capabilities toward the least restrictive environment and/or services.
- Initiate and coordinate professional assessments and share assessment/treatment data as authorized for implementation and coordination with other agencies, courts, and caretakers.
- Complete person-centered plans of service and periodic reviews of progress, ensuring the individual's preferences are reflected. Make direct or indirect contacts as required.
- Develop short‑ and long‑term goals and ensure implementation, coordination, and monitoring of plans in residential, employment, school, and other settings.
- Act as the primary advocate for assigned individuals and liaison with families, guardians, caretakers, and agencies involved in the service network.
- Provide consultation, referral, and in‑services to staff, caretakers, families, and other agencies as needed.
- Assist with placement planning, complete necessary paperwork, and coordinate contracts or other services. For specialized placement services, complete and coordinate residential treatment plans and conduct home visits as required.
- Provide community outreach to individuals with a history of hospital recidivism to prevent crisis situations.
- Participate in after‑hours on‑call, crisis intervention during the workday, and other duties as assigned.
- Engage in evidence‑based practices such as DBT, IDDT, FPE, IPS, etc.
- Provide care management services to veterans and their families as assigned.
- Complete OBRA assessments and provide OBRA coordination as directed by supervisor.
- Assist with grant requirements and ensure compliance with funding mandates.
- Coordinate and link integrated health care needs of individuals.
- Deploy as a CCBHC Care Manager to coordinate and link to external services.
- Perform initial intakes as assigned by the supervisor.
- Perform other duties as assigned.
- For Children's Care Management: link and coordinate services such as school, probation, DHHS; provide family psychoeducation; participate in the BTC committee as needed; serve populations 0‑21 years with SED or IDD diagnoses.
- For Adult Care Management: link and coordinate services such as SS, probation, DHHS, MRS, jail services; provide family psychoeducation; participate in the BTC committee as needed.
- For Behavioralist Care Management: link and coordinate services; provide behavioral intervention support to the residential home system; complete behavioral assessments, FBA, and behavioral plans per BTC regulations; set up data collection methods for providers; provide progress reports to providers, the treatment team, and BTC.
- Initiative, organization, and reasoning – self‑starter, problem solver, efficient time management, and adherence to policy.
- Ethics and teamwork – treats all with dignity and respect, collaborates effectively, communicates constructively, and works cooperatively as part of a team.
- Bachelor’s degree in Human Services or an accredited field.
- Preferred licensure: RN, LLBSW, LBSW, LLMSW, LMSW, TLLP, LLP, LLPC, or LPC.
- Required qualifications:
QMHP, QIDP, or CMHP appropriate to the population served. - Veteran status required when working with Veterans.
- For child care management, 40 hours of Children’s training per calendar year and CAFAS rater certification.
- Minimum three years of experience with examination, evaluation, and treatment of SED children and families.
- Substance license preferred; MCBAP development plan encouraged.
- BCaBA required for Care Manager – Behavioralist.
- Valid chauffeur’s license required when transporting clients.
- Strong written and verbal communication skills for reporting, correspondence, and presentations.
- Proficiency in Word Processing and a typing speed of 45 wpm.
- Mathematical skills – ability to compute rates, ratios, percentages, and interpret graphs.
- Reasoning ability – apply common sense and problem‑solving skills to written and oral instructions.
Regular duties include sitting, standing, and traveling between offices. Occasional lifting of up to 25 pounds. Vision requirements include close and distance vision with color and peripheral vision. Work environment may involve moderate noise and exposure to outdoor weather conditions.
