SUMMARY

This position is responsible for all functions of case management including advocacy, coordination assessment, linking, and monitoring of services for an assigned caseload of individuals with a primary diagnosis of developmental disability or mental illness. Settings include the CMH office, home, and/or the community.

ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Gather and review information such as medical, psychological, and social history contributing to the individual’s situation. Evaluate this information to determine the individual’s needs and capabilities toward a least restrictive environment and/or services.
  • Initiate and coordinate professional assessments, present assessment/treatment data as authorized, and collaborate with other agencies, courts, and caretakers.
  • Complete the person‑centered plan of service and periodic review of progress, ensuring that preferences of the individual are reflected. Make direct/indirect contacts, develop short‑ and long‑term goals, and ensure implementation and monitoring of the plan in residential, employment, school, and other settings.
  • Serve as primary advocate and liaison with families, guardians, caretakers, and agencies involved in their service network.
  • Provide consultation, referral, and in‑services to staff, caretakers, families, and other agencies as needed.
  • Assist with placement planning, complete required forms, and coordinate residential treatment plans. Visit homes to monitor progress when necessary.
  • Provide community outreach to individuals with a history of hospital recidivism to prevent crisis situations.
  • Participate in after‑hours on‑call, crisis intervention during the workday, and other duties as assigned.
  • Participate in evidence‑based practices such as DBT, IDDT, FPE, IPS, etc.
  • Provide care management services to veterans and their families.
  • Complete OBRA assessments and provide OBRA coordination as directed.
  • Assist with grant requirements and coordinate Integrated Health Care needs.
  • Coordinate and link external services for CCBHC Care Managers.
  • Perform initial intakes as assigned and any other duties as assigned.
SUB‑ROLE RESPONSIBILITIES
  • Children’s Care Manager: Link and coordinate services (e.g., school, probation, DHHS); provide family psychoeducation; participate in the BTC committee; work with populations 0‑21 years with SED or IDD diagnoses.
  • Adult Care Manager: Link and coordinate services (e.g., SS, probation, DHHS, MRS, jail services); provide family psychoeducation; participate in BTC committee.
  • Behavioralist Care Manager: Link and coordinate services; provide behavioral intervention support to residential home system; complete behavioral assessments, FBA, and behavioral plan; set up data collection methods; provide summaries to providers and BTC.
QUALIFICATIONS
  • Bachelor’s degree in Human Services Field (or equivalent) from an accredited institution.
  • Preferred licensure: RN, LLBSW, LBSW, LLMSW, LMSW, TLLP, LLP, LLPC, LPC, or SST licensure for Medicaid. Care Manager-Behavioralist requires BCaBA.
  • Qualify as QMHP, QIDP, or CMHP as appropriate for the population served.
  • Candidate must be a veteran or have served in active military and be discharged non‑dishonorably when working with veterans.
  • For work with children, require 40 hours of Children’s training per calendar year and be a CAFAS rater.
  • Minimum of three years of experience with examination, evaluation, and treatment of SED children and families.
  • Substance license preferred; staff encouraged to pursue MCBAP development plan.
