We are seeking a Licensed Social Worker, (0.2 FTE/ 16 hours per pay), in Osceola County at the Central Michigan District Health Department working in the Maternal Infant Health Support and other Public Health programs. Having excellent communication skills to accommodate people's social needs and concerns, while being detail oriented and possessing exceptional technical and counseling skill are necessary.

Bachelor's degree in Social Work (BSW) required, MSW preferred. Must be licensed to practice in the State of Michigan. One-year experience in providing service to families is required.

Salary Range:
Bachelor's degree (BSW) $27.17 - $33.96 / hour, Master's degree (MSW) $31.55 - $39.44 / hour.

Working independently and with minimal supervision, this position provides direct social services to individuals, families, and population groups in assigned program(s). Conducts professional planning and evaluation, and implements actions directed toward goals of prevention, assessment, risk reduction, and health status improvement. for individuals, families, and communities. Collaborates and consults with multidisciplinary and multi-agency teams to achieve desirable outcomes for families and communities.

• Promotes health of individuals, families, and communities through advocacy, education and/or counseling.
• Organizes and provides assigned program-specific social services and supports to individuals and/or families, directly or through referrals to appropriate services.
• Assess needs of clients, their situations, and support networks; plans, implements, and evaluates services to improve outcomes on an ongoing basis.
• Monitors and evaluates client progress according to measurable goals described in treatment and care plan. Documents plans, services, and outcomes utilizing program forms and/or electronic health records in an accurate and timely manner.
• Utilizes consultation data and social work experience to plan and coordinate client care, and initiates interventions, referrals, or advocacy as necessary to ensure service efficacy.
• Collaborates with health care providers and community partners, as well as other health department staff, to improve individual and population health.
• Participates in mentoring new staff and allied health professional students.
• Maintains professional knowledge, and keeps current on assigned program requirements, regulations, protocols, and practices to ensure continual compliance and quality improvement.
• Performs other duties and responsibilities as necessary in response to emergencies, workloads, or staffing shortages. Works flexible hours as needed.

• Participates in Emergency Preparedness Incident Command System (ICS) as assigned.

• Maintains up-to-date vaccines, including flu (influenza), in accordance with CDC guidelines:
  Recommended Vaccines for Healthcare Workers, in which you allow CMDHD to access your current immunization status based on Michigan Improvement Care Registry (MCIR).

• Knowledge of social work theories, principles and practices acquired through completion of Bachelor’s degree from accredited school of school work.
• Advanced social work skills and knowledge of public health policies, procedures, and practices at a level normally acquired through two years of related, professional experience.
