Location: Linden
Dept of State Health Services, Region 4/5, Linden, TX, seeks a Social Worker II to provide complex public health social work and comprehensive case management for children with special health care needs and their families. Duties include home visits, intake and assessments, coordinating with medical providers and community partners, and documenting case activities.
Requires current Texas social work license and a CSWE‑accredited degree;
Master’s degree preferred. Travel up to 50% is required.
