Social Worker - Hospice

Our client is a well-established Hospic e organization. They have a current opening for a Hospice Social Worker for their Middleborough, MA (south territory) and surrounding areas. Hospice experience is ideal but Social Workers with at least one year of experience in another setting and an interest in Hospice care will be considered.

• At least one year of Social Worker experience in any setting. Hospice experience is strongly preferred but not required.
• Massachusetts Licensed Clinical Social Workers is required.
• Connect families with local resources as needed.

• Ensure psychosocial needs of the patient and their families are met.
• Ensure compliance with all state and federal regulations.
• Participate in Interdisciplinary team (IDT) meetings.
