Job Summary

The Home Health Registered Nurse (RN) plays a critical role as the agency's first responder, delivering nursing care to clients across Bright Star Care's service area, including Cuyahoga, Lorain, and Medina counties.

The RN ensures continuity of care during urgent coverage needs, new client onboarding, and high‑priority cases. The role is dynamic and requires flexibility, proactive communication, and a commitment to excellence in clinical care and interpersonal skills.

Rapid Response & Readiness

• Full‑time role: 40 hours per week
• Hours are completed within the following flexible working/on‑call window:
  Monday‑Friday and every other weekend, 7:00am‑11:00pm
• Weekly schedule varies based on patient needs
• Overtime available depending on case volume and staffing
• Must be prepared to leave for a visit or shift within 30‑60 minutes
• Maintain readiness by ensuring uniform and  are worn, childcare is pre‑arranged, cell phone is charged and ringer is on, transportation is available, and equipment and supplies are accessible
• Support Bright Star office operations when not assigned to field shifts

Direct Client Care & Case Management

• Conduct initial assessments, re‑assessments, and safety evaluations
• Deliver skilled nursing per Ohio Nurse Practice Act (e.g., infusion therapies, wound care, injections)
• Observe and report changes in client condition, documenting thoroughly
• Follow infection control protocols and universal precautions
• Respond appropriately to emergencies and use equipment safely

Administrative Support

• Develop and maintain person‑centered Plans of Care and obtain physician signatures
• Coordinate care with families, providers, and internal staff
• Assist with referral intake and follow‑up communications
• Assist Clinical Director with weekly chart reviews for compliance, incident reporting, client discharges, and daily documentation review to support billing and reimbursement
• Supervise and coach nursing/caregiving staff, including competency assessments, supervisory visits, caregiver‑client introductions, field education, orientation, skills training, and in‑service programs, and be available for urgent clinical issues or questions

Licensure & Education

• Current, unrestricted Registered Nurse (RN) license in the State of Ohio
• Degree in Nursing from a National League for Nursing‑accredited program
• Minimum of three (3) years clinical experience, including at least one (1) year of documented case management
• Preferred:
  One (1) year of experience in a home health agency and healthcare management
• Preferred:
  One (1) year of experience working with pediatric clients

Clinical & Regulatory Competencies

• Strong nursing assessment skills and attention to detail
• Ability to supervise, educate, and direct staff effectively
• Thorough understanding of regulatory compliance at state and federal levels
• Familiarity with Joint Commission policies and survey preparedness
• Adherence to HIPAA standards and commitment to client confidentiality
• Ability to manage complaints and complex situations using sound clinical judgment
• Experience in client teaching and public speaking

• Strong interpersonal skills including active listening, empathy, leadership, and motivation
• Ability to multi‑task, troubleshoot, and remain composed in high‑pressure situations

• Technologically proficient (Microsoft Office, EMR platforms)

Availability & Health Requirements

• Flexible schedule with availability for on‑call coverage within assigned territory and surrounding region
• Valid driver’s license, reliable transportation, and current auto insurance
• Willingness to travel daily throughout the assigned territory, including weekends, evenings, and holidays
• Current CPR certification and negative TB test within the past 12 months
• Up‑to‑date vaccinations:
  Influenza, COVID‑19, Tetanus, Diphtheria, Pertussis, Rubella, Varicella, MMR, Hepatitis B
