Experienced LPN | Trach & Vent Care Specialist Home Care

Schedule

2:00 PM – 11:00 PM | Monday–Friday (with some weekends). Some flexibility in hours.

• Provide trach and ventilator care in patients' homes under RN supervision.
• Assist in implementing individualized care plans and follow care instructions.
• Monitor patient condition and promptly report changes to the RN or Director of Nursing.
• Maintain accurate, timely documentation in the home care setting.
• Follow agency protocols, safety standards, and HIPAA compliance.
• Educate patients and families on home trach and ventilator care.
• Collaborate with the home care team to support optimal patient outcomes.
• Participate in ongoing training and professional development.

$30‑$40/hr plus weekly direct deposit & overtime.

• Active Ohio RN license (unencumbered).
• 1+ year clinical experience (home care/critical care preferred).
• Strong clinical and independent decision‑making skills.
• BLS/CPR certified + TB clearance.
• Reliable transportation & valid driver's license.
• Ability to lift up to 50 lbs and assist with patient mobility.
• Up-to-date vaccinations (COVID-19, Flu, Hep
  
  B).
