The Home Health Registered Nurse (RN) plays a critical role as the agency's first responder, delivering nursing care to clients across Bright Star Care's service area, including Cuyahoga, Lorain, and Medina counties. This nurse ensures continuity of care during urgent coverage needs, new client onboarding, and high priority cases. While some shifts may be consistent, the role is inherently dynamic and requires flexibility, proactive communication, and a commitment to excellence in both clinical care and interpersonal skills.
This position will be a good fit for someone who is flexible, hard-working and very creative in solving problems; someone who doesn't mind rolling up his/her sleeves and does whatever it takes to take care of our people!

Job ResponsibilitiesRapid Response & Readiness
- Full‑time role: 40 hours per week
- Hours are completed within the following flexible working/on‑call window:
- Monday-Friday and every other weekend, 7:00am-11:00pm (This is not a single long shift - it reflects the range of hours when visits or shifts may be scheduled, or when on‑call flexibility may be required.)
- Weekly schedule varies based on patient needs
- Overtime available depending on case volume and staffing
- Must be prepared to leave for a visit or shift within 30-60 minutes (Bright Star will always schedule visits and shift as far in advance as possible.)
- Maintain readiness by ensuring:
- Uniform and are worn
- Childcare is pre-arranged
- Cell phone is charged, and ringer is on
- Transportation is available
- Equipment and necessary supplies are always accessible
- Support Bright Star office operations when not assigned to field shifts
Direct Client Care & Case Management
- Conduct initial assessments, re‑assessments, and safety evaluations
- Deliver skilled nursing per Ohio Nurse Practice Act (e.g., infusion therapies, wound care, injections, etc.)
- Observe and report changes in client condition, document thoroughly
Administrative Support
- Develop and maintain person‑centered Plans of Care and obtain physician signatures
- Coordinate care with families, providers, and internal staff
- Assist with referral intake and follow‑up communications

- Weekly chart reviews for compliance (JCAHO, state, brand standards)
