Registered Nurse (RN) – Home Health Care | Flexible Schedules | Home & Facility-Based Care

We are seeking a skilled Registered Nurse (RN) with Trach & Vent experience to provide high-quality, specialized patient care in home and facility-based settings. • Weekend Support (AM or PM)
• It can be a PT or PRN shift

• Someone who has experience in Trach and Vent | Pediatric Care
• Supportive, hands-on clinical environment
• Meaningful one‑on‑one patient care
• Experience in pediatric, memory/dementia, and hospital‑level care

• Provide skilled nursing care for pediatric, memory/dementia, and complex care patients
• Perform and manage Trach & Vent care, including airway and ventilator support
• Monitor patient condition and respond to clinical changes appropriately
• Assist with ADLs and patient mobility as needed
• Implement infection control and patient safety protocols
• Administer care per physician orders and clinical guidelines
• Maintain accurate, timely clinical documentation
• Communicate effectively with care team, patients, and families
• Ensure compliance with all safety, hygiene, and clinical standards
• Participate in ongoing training and skills development

• Active Ohio RN license (unencumbered)
• Minimum 1 year clinical experience (home care or critical care preferred)
• Strong clinical judgment and independent decision‑making skills
• Current BLS/CPR certification and TB clearance
• Reliable transportation & valid driver's license
• Ability to lift up to 50 lbs and assist with patient mobility
• Up‑to‑date vaccinations (COVID‑19, Flu, Hep
