LPN : Trach & Vent Care – Specialized Team Home Care

Full Time LPN with Trach & Vent Experience in Home Care

We’re looking for a skilled Licensed Practical Nurse (LPN) with Trach & Vent experience to provide high-quality, specialized patient care in a supportive home‑care environment.

Schedule: 2:00 PM – 11:00 PM (Monday–Friday, with occasional weekend work; flexible hour options).

• Provide trach and ventilator care in patients’ homes under RN supervision.
• Assist in implementing individualized care plans and following care instructions.
• Monitor patient condition and promptly report changes to the RN or Director of Nursing.
• Maintain accurate, timely documentation in the home‑care setting.
• Follow agency protocols, safety standards, and HIPAA compliance.
• Educate patients and families on home trach and ventilator care.
• Collaborate with the home‑care team to support optimal patient outcomes.
• Participate in ongoing training and professional development.

• Provide skilled trach and ventilator care within RN scope of practice.
• Implement individualized care plans using clinical judgment.

• Active Ohio RN license (unencumbered).
• 1+ year clinical experience (home care or critical care preferred).
• Strong clinical judgment and independent decision‑making skills.
• BLS/CPR certified with TB clearance.
• Reliable transportation and valid driver’s license.
• Ability to lift up to 50 lbs and assist with patient mobility.
• Up‑to‑date vaccinations (COVID‑19, Flu, Hep
