Job Description:
Speech Language Pathologist (SLP) PRN Home Health Join Elara Caring and make a difference where it matters most, Elara Caring, we believe the best care happens where patients feel most comfortable. Every day, our teams provide high-quality care to more than 60,000 patients in their preferred setting. We are seeking a PRN Speech Language Pathologist to serve patients in the Middlebury and Danbury area.
You will have the opportunity to provide personalized care while working with a collaborative and supportive team.
- Evaluate patients and develop individualized plans of care.
- Provide speech and language therapy services according to physician orders.
- Help patients improve their communication, cognitive, and swallowing abilities.
- Educate patients and caregivers on home therapy programs.
- Maintain accurate and timely clinical documentation.
- Follow OASIS, PPS, and company guidelines.
- Collaborate with physicians and interdisciplinary team members to ensure continuity of care.
- Master’s degree in Speech Language Pathology
- Current and unrestricted SLP license in the state of practice
- At least one year of clinical experience as a Speech Language Pathologist
- Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
- Ability to travel throughout the Middlebury and Danbury coverage area
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment

Compensation for this role will be determined based on a variety of factors, including qualifications, skills, competencies, and relevant experience.
The base salary for this position is $70.00 to $80.00 per point, based on the company’s good faith estimate at the time of posting. Actual pay will be determined based on factors such as education, experience, skills, and internal equity.
