Helen Porter Rehabilitation & Nursing is a welcoming, secure residence in Middlebury, Vermont. It offers skilled nursing care and assistance with daily living in a home‑like environment for 98 residents. The facility is licensed by the state, certified by Medicare and Medicaid, and provides intermediate care.

• Long Term Care Unit – provides nursing care, daily living assistance and tailored programs for residents.
• Memory Care Unit – specialized care for individuals with dementia, supporting dignity and functional ability.
• Post Acute Care Unit – goal‑directed care after surgery or hospitalization, with therapy and support to maximize independence. Conditions include strokes, neurological conditions, orthopedic care, respiratory conditions, and cardiac and post‑trauma cases.

• Current RN licensure recognized by the State of Vermont.
• Appropriate experience in a specific clinical area. Varies by unit.
