This position is located in Middlebury, VT.

Assignment Duration:
This is a 13‑week contract position.

Shifts will consist of 4×8‑hour shifts one week and 5×8‑hour shifts the next, with a minimum of every other weekend required.

• Deliver high-quality patient care in a long-term care environment.
• Monitor patient health, administer medications, and maintain accurate records.
• Provide support to patients with various long-term care diagnoses.
• Collaborate with a multidisciplinary team to ensure optimal patient outcomes.
• Float to Memory Care and PAC as needed.

• A minimum of 1 year of clinical experience is required.
• Prior travel experience is preferred.
• Experience with the EPIC documentation system within the past 2 years is required.
• Dementia capable care certification is preferred.
