Responsibilities
  • Deliver direct nursing care to residents on the post-acute/long-term care unit, adhering to care plans and policies.
  • Monitor patient status, record vital signs, assessments, and progress notes; escalate changes per protocol.
  • Administer medications and treatments as ordered, using the EMR (EPIC) for documentation.
  • Collaborate with the interdisciplinary team to develop, implement, and update resident care plans.
  • Assist residents with activities of daily living and mobility; provide safe, respectful patient-centered care.
  • Respond to call lights and emergencies; ensure safety and comfort of residents; participate in rounds and discharge planning as needed.
  • Float as required to meet unit needs; maintain competency across assigned areas.
Requirements
  • Current Licensed Practical Nurse license in Vermont (LPN).
  • Minimum 1 year of practical nursing experience.
  • Prior travel nursing experience required.
  • EPIC EMR experience within the past 2 years is required.
  • Dementia-capable care certification preferred.
  • BLS certification required.
  • Ability to work Night shifts; willingness to float as needed.
  • Reliable transportation is required (personal vehicle or rental strongly recommended).
