LVN ​/ LPN – LTC​/SNF

Licensed Practical Nurse (LPN)

Seeking a Licensed Practical Nurse (LPN) for a post-acute care unit to provide skilled nursing services to long-term and rehab patients. The role involves patient care, documentation, and occasional floating to other units as needed.

Responsibilities:

• Administer medication and treatments using EPIC system documentation.
• Monitor and assess patient conditions, including IV therapy and respiratory support.
• Document care activities accurately and timely, ensuring compliance with facility protocols.
• Collaborate with the healthcare team to develop and implement patient care plans.
• Float to different units as required, supporting staffing needs.
• Ensure compliance with all safety standards and infection control procedures.

Qualifications:

• Valid LPN license with active file on record.
• Minimum of 1 year of LPN experience, preferably in LTC/SNF settings.
• Experience with EPIC electronic medical records within the past 2 years.
• Prior travel experience required.
• Dementia-capable care certification preferred.
• IV certification is highly preferred.
