Overview

Licensed Practical Nurse (LPN) – Long Term Care (Post-acute skilled nursing rehab) in Middlebury, Vermont. Travel temporary assignment starting 08/20/2026 for approximately 4 weeks, 3x12 Night shifts. Weekly pay between $1,342 and $1,440 with no guaranteed hours. Work setting is a long-term care/nursing home unit requiring float coverage as needed.

Responsibilities
  • Administer prescribed medications and treatments, including IV therapies where certified
  • Monitor patient status, perform head-to-toe assessments, document findings, and respond to changes in condition
  • Perform phlebotomy, EKGs, basic transports, and related procedures as trained
  • Maintain accurate documentation in EPIC, including medication administration records and care plans
  • Collaborate with the care team (LPNs, CNAs, unit clerk) to ensure safe, efficient post-acute rehab care
  • Provide patient and family education as needed and maintain a patient-centric, dignity-focused approach
Requirements
  • Active LPN license in Vermont (or eligibility to practice); license must be on file at submission
  • Minimum 1 year of clinical experience; prior travel experience preferred
  • BLS certification at time of assignment
  • EPIC experience within the past 2 years required
  • IV certification preferred for LPNs; dementia-capable care certification preferred
  • Education:

    Practical Nursing Diploma from an accredited program
  • Ability to work night shift, float as needed, and adhere to unit policies in a post-acute, skilled nursing rehab setting
