Helen Porter Rehabilitation & Nursing, part of the University of Vermont Health Network and Porter Medical Center, is seeking a compassionate and dedicated Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our Memory Care Unit on the day shift.

Job Title: Registered Nurse (RN) or Licensed Practical Nurse (LPN)
Location: Helen Porter Rehabilitation & Nursing, Middlebury, VT
Schedule: Full-Time, 36 Hours per Week
Shift: Days, 6:45 AM - 3:15 PM
Weekend Requirement: Rotating Weekends

What You ll Do

As an RN or LPN on our Memory Care Unit, you will:

  • Provide skilled nursing care to residents with Alzheimer s disease and other memory-related conditions
  • Assess, monitor, and document resident health status and changes in condition
  • Administer medications and treatments according to physician orders
  • Promote resident safety, comfort, and well-being
  • Collaborate with nursing assistants, providers, rehabilitation staff, and other members of the care team
  • Communicate effectively with families and caregivers
  • Support person-centered care plans that honor each resident s preferences and needs
  • Maintain accurate clinical documentation and ensure compliance with regulatory standards
  • Contribute to a positive, compassionate, and team-focused culture
QualificationsRN Candidates
  • Current Vermont Registered Nurse (RN) license or eligibility for Vermont licensure
LPN Candidates
  • Current Vermont Licensed Practical Nurse (LPN) license or eligibility for Vermont licensure
Preferred Qualifications
  • Experience in memory care, dementia care, long-term care, skilled nursing, assisted living, rehabilitation, hospital, or acute care settings
  • Strong assessment and critical thinking skills
  • Excellent communication and interpersonal abilities
  • Commitment to delivering compassionate, resident-centered care
  • Ability to work effectively as part of a collaborative team
