The University of Vermont Health Network seeks a compassionate Licensed Nursing Assistant to join our Helen Porter Rehabilitation and Skilled Nursing long term care team on full time nights. In this role, you will provide hands on bedside support with ADLs, mobility, toileting, and comfort measures, accurately obtain vital signs, and assist nurses with treatments and documentation in the EHR. You'll promote safety, dignity, and infection control while supporting primarily geriatric residents.
You'll work with in a collaborative, academic health system that values learning, inclusion, and professional growth.
Responsibilities
- Provide direct resident care including hygiene, grooming, toileting, feeding, and mobility support on night shift
- Measure and record vital signs, intake/output, and report changes in condition to the nurse
- Assist with safe transfers, repositioning, and use of lifts and mobility devices to prevent falls and injury
- Maintain infection control, cleanliness, and a safe, respectful environment in resident rooms and common areas
- Document care and observations accurately in the electronic health record
- Support nurses with basic treatments, specimen collection, and monitoring as directed
- Respond promptly to call lights and resident needs, promoting comfort, dignity, and person-centered care
- Collaborate with RNs, LPNs, therapists, and families to support individualized care plans for long-term care residents
