LVN​/LPN – LTC​/SNF

Job Summary:

Seeking an experienced Licensed Practical Nurse (LPN) for a post-acute care unit. The ideal candidate will provide skilled bedside care, supporting patient recovery and well-being in a long-term care setting.

Responsibilities:

• Administer medications and treatments accurately per physician orders.
• Monitor patient vital signs and observe for changes or adverse reactions.
• Document patient information and care activities within the EPIC system.
• Collaborate with multidisciplinary team to ensure comprehensive patient care.
• Assist with patient transfers, mobility, and daily activities as needed.
• Adhere to infection control protocols and facility guidelines.

Qualifications:

• Valid VT LPN license in good standing at the time of submission.
• Minimum of 1 year of clinical experience in a post-acute or long-term care environment.
• Prior travel experience preferred.
• EPIC EMR experience within the past 2 years is required.
• Certification in IV therapy is preferred.
• Ability to work every other weekend or as scheduled, including holidays.
