Requirements
- Active LPN license required if applicable
- Minimum 1 year of clinical experience required
- EPIC EMR experience within the past 2 years required and nonnegotiable
- IV certification preferred
- Dementia capable care certification preferred
- Reliable personal transportation required due to lack of public transit access, with all-season or snow tires recommended November through April
- Post-acute skilled nursing rehab unit within a long-term care nursing and rehabilitation facility
- Patient population aged 60+ requiring skilled nursing rehab care
- Nurse to patient ratio of 1:14 with ancillary staff support including 3 LNA/CNA/PCT per shift and a unit secretary on day shift
- Documentation and medication administration managed through EPIC
- Uses IV pumps and performs phlebotomy, EKGs, transports, nebulizer to inhaler conversions, and IV starts
- 12-hour day shifts scheduled Monday through Sunday with varied scheduling and no guaranteed days off
- Minimum every other weekend requirement with schedule adjusted to meet unit needs
- Must be able to work all holidays
- Occasional/as-needed floating to long-term care and memory care units
- Responsibilities include medication administration, phlebotomy, EKGs, patient transports, nebulizer to inhaler conversions, and IV starts
- Works alongside ancillary staff including LNA/CNA/PCT and unit secretary support on day shifts
- Bi-weekly orientations held on Mondays
- First-time travelers may be considered
