Job Description

Swan Hospice is actively seeking Registered Nurses (RN) to join our team in New Jersey.

We provide quality hospice care to patients in skilled nursing facilities, assisted living facilities and in private homes.

Responsibilities
  • Oversee primary caseload of hospice patients, delivering skilled professional nursing care to patients, families, and caregivers as prescribed by the physician and in compliance with Medicare Conditions of Participation, New Jersey hospice licensure laws, accrediting body standards and agency policies and procedures.
  • Provide the initial and comprehensive nursing assessment and identification of problems for each patient; reassess nursing needs and collaborate to develop the hospice interdisciplinary team (IDT) plan of care.
  • Implement and continually reevaluate and revise the plan of care to meet the patient's evolving nursing needs.
  • Educate, supervise, and counsel patients, family members, and staff regarding nursing care, patient needs, and related problems at home.
  • Work closely with other IDT members to ensure all patient and family care needs are met.
  • Supervise hospice aides/homemakers.
Qualifications
  • Graduate of an accredited registered nursing program.
  • Bachelor’s degree in nursing preferred.
  • Certified Hospice and Palliative Nurse (CHPN) certification preferred.
  • Minimum one (1) year general nursing experience preferred, with current hospice, home health, medical, surgical, or critical care experience.
  • Experience with pain and symptom management interventions preferred.
  • Ability to work within an interdisciplinary setting.
  • Understanding of hospice philosophy and principles of death/dying.
  • Registered Nurse licensed by the New Jersey State Board of Nursing in accordance with N.J.S.A. 45:11-23 et seq. and N.J.A.C 13:37.
  • Valid driver’s license and reliable insured transportation meeting New Jersey state requirements.
  • Satisfactory references from employers and/or professional peers.
  • Satisfactory criminal background check.
