Overview
GENERAL FUNCTION
Join our dedicated team in making a real difference in the lives of individuals across the lifespan in Wood County. As a Service Facilitator, you’ll play a key role in supporting people living with mental health and/or substance use challenges on their journey to recovery and independence. You’ll provide personalized services that include education, advocacy, and ongoing support—tailored to each individual’s unique needs and goals.

Wage: $62,344.20 to $70,504.85/annually
Wage as of 7/1/26: $63,593.40 to $71,915.35/annually
Responsibilities
Empower Individuals on Their Recovery Journey — Deliver personalized, recovery-oriented case management services to children and adults experiencing mental health and/or substance use challenges. Collaborate with individuals and their recovery teams to assess needs, build integrated service plans, and provide ongoing support. Use a strengths-based approach to foster hope, independence, and empowerment; both one-on-one and in group settings.Ensure Quality and Compliance — Complete essential documentation in line with DHS 36 and Medical Assistance guidelines, including assessments, treatment plans, progress notes, and eligibility screenings. Review service provider documentation and invoices to ensure accurate billing and compliance.Be a Valued Team Member — Work as part of a dynamic multi-disciplinary team. Participate in recovery team meetings and contribute to holistic, person-centered care for consumers enrolled in CCS (Comprehensive Community Services).Facilitate Real-Life Skill Building — Lead and support psychosocial rehabilitation activities, including daily living and community living skills, recreational and educational groups; designed to help individuals thrive in real-world settings using community-based resources.Promote a Safe and Supportive Environment — Prioritize safety in all aspects of your work by following county safety policies and performing duties in a manner that protects yourself, your team, and the individuals you serve.Lead with Respect and Integrity — Represent Wood County’s Core Values through respectful, ethical, and professional interactions.
