Nurse Practitioner - Adult Op Mental Health Services
Middlesex Health is seeking a compassionate and skilled Psychiatric Advanced Practice Registered Nurse (APRN) to join our expanding Adult Outpatient Mental Health Services team. This (35 hours/week) position offers the opportunity to provide high-quality psychiatric care within a collaborative, multidisciplinary environment dedicated to delivering exceptional patient outcomes. As a Psychiatric APRN, you will have the unique opportunity to support continuity of care across the mental health services continuum.

Responsibilities
- Conduct psychiatric evaluations and diagnostic assessments for adult patients
- Develop and implement individualized treatment plans
- Prescribe and manage psychiatric medications in accordance with state and federal regulations
- Provide ongoing medication management and psychiatric follow-up care
- Collaborate with therapists, social workers, primary care providers, and other members of the treatment team
- Maintain timely and accurate clinical documentation within the electronic health record
- Participate in quality improvement initiatives and support a patient-centered model of care
Qualifications
- Graduate of an accredited school of nursing
- Current Connecticut APRN license
- Board certification as a Psychiatric-Mental Health Nurse Practitioner (PMHNP) or eligibility to obtain certification
- Experience working with mental health populations in outpatient and/or inpatient settings
- Strong interpersonal, communication, and clinical assessment skills
- Commitment to providing outstanding patient care and promoting a collaborative work environment
