Primary Care Nurse Practitioner​/Physician Assistant

Hartford Healthcare seeks a Primary Care Nurse Practitioner / Physician Assistant to deliver comprehensive, patient-centered primary care in our Healthcare & Medical Services department. In this role, you will perform assessments, diagnose and manage acute and chronic conditions, prescribe medications, and coordinate care within a multidisciplinary team. You will partner closely with physicians, nurses, and specialists to provide evidence-based, high-quality care for diverse patient populations.
