A residential step-down program where pregnant and/or parenting women are required to participate in substance use community treatment while living in a safe and structured environment. Clients will meet with their Case Manager on a weekly basis and attend daily psycho educational groups. The program is designed to support women as they return to their communities as healthy and productive parents in recovery.
PositionSummary
The Case Manager 1 is responsible for delivering case management services to the Women & Children Substance Use Treatment Facility. Duties include intake assessments, development and implementation of individual care plans, liaison with treatment providers and community resources, facilitation and documentation of client and group activities, and daily psychoeducation groups in accordance with Women’s and Children’s curriculum and DMHAS core topics.
Responsibilities- Conduct intake assessments and develop individualized care plans.
- Coordinate and liaise with treatment providers and community resources.
- Facilitate and document client and group activities in the program.
- Lead daily psychoeducation groups following the Women’s and Children’s curriculum.
- Maintain accurate case documentation and comply with agency policies.
- Some post secondary education.
- Minimum 2 years of relevant experience.
- Valid Connecticut Driver’s License.
