Ohio Health in Ohio seeks a Speech-Language Pathologist for Home Care in the Outpatient Rehab department. You will evaluate patients, develop and carry out plans of care, provide family instruction, and coordinate discharge planning with a multidisciplinary team to achieve optimal functional outcomes.
You will document progress, communicate with physicians, and supervise SLP students and CFY placements while contributing to department quality and safety initiatives. Day shift; 40 hours per week.
