4 10‑hour days

Minimum of two years’ experience as a primary care provider preferred

Position Summary

Clinician member of the Multi Care Medical Partners multidisciplinary team delivering population‑based care. The ARNP will provide comprehensive primary care services focusing on prevention, chronic disease management and health promotion for diverse patient populations. This role requires collaboration with physicians, RNs, pharmacists, social workers, behavioral health specialists and other professionals to optimize the health of Multi Care communities.

Responsibilities
  • Direct Patient Care
    • Conducts comprehensive assessments, diagnoses conditions, develops plans, and prescribes medications for patients across the lifespan
    • Diagnoses and treats acute and chronic disease using evidence‑based guidelines
    • Provides preventive care, health education and counseling to individuals and families
    • Timely and accurate documentation, inclusive of direct patient encounters and asynchronous care
    • Participates in the coverage for clinic colleagues per departmental guidance
    • Participates in after‑hours call coverage per departmental guidance
  • Multidisciplinary Collaboration
    • Participates in multidisciplinary meetings, care reviews and care planning
    • Coordinates care with all members of the delivery team
    • Contributes to the development and improvement of population‑based care
  • Population Health Management
    • Utilizes data and risk stratification tools to prioritize patients at greatest risk for outreach
    • Engages in quality improvement focused on health outcomes, cost effectiveness and cost reduction
    • Supports transitions of care and reduced readmissions and ED use
  • Citizenship
    • Demonstrates the Multi Care values of compassion, stewardship, kindness, joy, collaboration and respect
    • Adheres to Multi Care policies and processes
Requirements
  • Maintains active Washington State Nurse Practitioner or Physician Assistant license
  • DEA and state prescriptive authority
  • Current BLS for Healthcare Providers certification by the American Heart Association

Pay and Benefit Expectations

Pay is influenced by factors specific to applicants, including but not limited to skill set, level of experience, and certification(s) and/or education.
