Reentry Navigator (Case Manager)-Georgetown, Delaware

Essential functions of the Reentry Navigator to include:
Capacity Building, Asset Mapping, Community Outreach and Engagement, Develops framework for Reentry Court, Assessment, Individual Achievement Planning, Benefit Eligibility Enrollment, traditional case management functions (housing referrals, transportation coordination, health care referrals, criminal justice liaison etc.) & delivery of phase-specific TCU Cognitive Behavioral Sessions (CBI). They correspond with families and outside organizations for client support post-release.

  • Provide comprehensive case management services for a caseload of high-risk men (participants) who return to the Greater Georgetown community from incarceration. Caseload: 25 - 30 participants.
  • Assess participant needs using validated risk assessments and deliver adequate and effective dosage of cognitive behavioral treatment and indicated ancillary services.
  • Create individualized case management plans (Achievement Plans) that match a participant’s level of criminogenic risk and need.
  • Conduct individual case-management sessions at least once per week for each participant. (or as agreed to with Clinical Director.)
  • Assess changes in participant’s needs and responsivity factors at regular intervals and adjust level of care.
  • Utilize incentives, sanctions, and therapeutic adjustments as appropriate.
  • Provide/develop case management services for families that include parenting training.
  • Evidence-Based CBI Interventions

Proficiently deliver cognitive behavioral, criminal-thinking intervention groups that are standardized, validated, and have been demonstrated to improve outcomes for individuals who are at a high-risk to re-offend.

Services to Target Participant Needs

Provide a continuum of ancillary services that target the criminogenic needs and responsivity factors of participants.

Ancillary services may include job skill training, employment referrals, housing assistance, peer support services, and introduction to pro-social environments.

Connect participants to human service benefits, mental health treatment, trauma treatment, and substance abuse treatment services.

Reentry Court Status Hearings & Communication

Attend bi-weekly Re-entry Court status hearings with the participant and the Re-entry Court team (specialized judge, probation officers, and other ancillary service providers).

Attend pre-court staff meetings to provide information and professional perspective regarding program participants progress.

Consistently participate in the status hearings to demonstrate the collaborative nature of the program to the participants.

Communicate regularly with Re-entry Court team outside of status hearings

Must have strong writing ability to communicate clearly and concisely with the court.

Data and Evaluations

Record information concerning the provision of services for participants (case notes).

Input data in a timely and reliable manner for use in case and program management.

Examine outcomes for all eligible participants who enter the program regardless of whether they graduated, withdrawn, or were terminated.

Review statistics relevant to program performance and implement policy adjustments and training when necessary.

Qualifications

Preferred:
Licensed Clinical Social Worker, LPC, LCDP and certification as an addiction professional (CADC, LCDP with master’s degree in human services/social work or other related field).

At least three years of experience performing intensive social service case management

At least three years of experience in a supervisory role.

Ability to motivate and establish therapeutic alliance with clients

Knowledge of, and demonstrated skills in, clinical counseling such as motivational interviewing and strength-based case management, as well as training in conducting process and didactic group sessions using cognitive behavioral therapy

Ability to coordinate and provide services to clients utilizing a case plan

Computer literacy required (ability to compose and send emails, draft client case notes, use an internet-based database system, compose documents in Microsoft Word, and perform basic internet research)
