Soleo Health is seeking a Per Diem Infusion Nurse to support our infusion patients in Middletown, OH and surrounding areas. Join us in Simplifying Complex Care!

• Provide direct care for all assigned patients, including initiation and maintenance of communication with appropriate care team members (pharmacists, prescribers, dietitians, home patient representatives, care partner, and community resource staff).
• Evaluate and educate patient/care partner to safely administer home infusion therapy.
• Evaluate the home environment and identify interventions required to make it safe and appropriate.
• Develop in conjunction with other team members the plan of care and evaluate response to therapy at each encounter.
• Manage schedule via the company‑approved platform per company policy; communicate any scheduling changes promptly.
• Coordinate patient care via the approved company platform.
• Ensure continuous nursing support services to our customers.
• Document all patient encounters, communications, and care activities at the time of service per Soleo policy and regulatory requirements.
• Maintain strong knowledge of Soleo therapies, protocols, and clinical standards.
• Ensure timely submission of plans of treatment (POT) and required clinical documents per policy and state guideline.
• Work with appropriate departments to ensure all required patient documents (consents, POTs) are completed and returned to the branch office within an acceptable timeframe.

• Must be able to work per diem 18‑22 hours per week, days and weekends as needed after 2 pm (must have 3–4 days of partial availability to see patients).
• Must be knowledgeable of IV access devices, including IV lines, accessing ports, and administration of infusion medications.
• Must be able to drive up to 2 hours one direction to see patients (mileage reimbursement and hourly rate of pay offered).
• Must be able to provide infusions in patient homes.
• Must be comfortable working in patient homes with animals or cigarette smoke.
• Must hold a single state RN license in good standing for Ohio.

• Current RN license by the applicable state’s Board of Nursing, in good standing—Ohio RN license.
• RN degree; bachelor’s degree in nursing (BSN) from an accredited institution preferred.
• Advanced certification in specialty areas of practice (e.g., CRNI, ONS, IgRN) preferred but not required.
• Minimum 3 years nursing experience with background in infusion therapy preferred.
• Experience in ICU and/or Cardiac Care preferred.
• Proficient with Microsoft Outlook and Microsoft Office Suite.
• Valid state‑issued driver’s license with no significant restrictions.
