Most outpatient nursing jobs turn into phone tag, prior auth queues, and rooming patients you never speak to again. This is not that role. Here you carry a panel you know, you are in the room for the treatments that matter — long-acting injectables, SPRAVATO monitoring, medication-assisted treatment — and you have direct access to the prescriber instead of a message queue.
SOS Wellness is a CARF-accredited, OhioMHAS-licensed, LegitScript-certified behavioral health practice led by Medical Director Irfan Dahar, MD, who brings more than 30 years in community mental health and addiction medicine. We treat psychiatric and substance use disorders together, in a trauma-informed model, across outpatient psychiatry, therapy, and an Intensive Outpatient Program.
What you would doRN and LPN alike- Long-acting injectables. Administer and document LAI antipsychotics and naltrexone injections.
- Clinical monitoring. Vitals, point-of-care testing, specimen collection, and metabolic monitoring.
- Medication checks. Structured medication reviews — adherence, tolerability, and what the patient is actually taking versus what the chart says.
- Side effect monitoring. Screen for and document adverse effects — metabolic, extrapyramidal, sedation, GI — and accelerate to the prescriber the same day.
- Patient well checks. Scheduled check-ins between prescriber visits, so changes get caught early instead of at the next appointment.
- Health histories. Collect and update medical, psychiatric, substance use, medication, and allergy histories that the prescriber actually relies on.
- Medication education. What it does, what to expect, and what to call about — in plain language patients actually retain.
- Prior authorization and pharmacy coordination. Worked alongside the prescriber, not dumped on you alone.
- Same-day prescriber communication. You raise a concern, you get an answer that day.
- Nursing assessment and triage. First clinical judgment on same-day patient calls and walk-in concerns.
- SPRAVATO. REMS-compliant administration and post-dose observation as the service grows.
- Addiction nursing at Middletown. Withdrawal monitoring and medication-assisted treatment support within our accredited addiction program.
- Nurse-led IOP education groups. Sleep, nutrition, medication adherence, and the physiology of relapse — your curriculum, your room.
- Standardized clinical monitoring. AIMS and other structured assessments feeding directly into prescriber decisions.
- Behavioral health, addiction, or med-surg background — psychiatric experience preferred, not required
- Comfortable exercising independent clinical judgment without a chain of approvals
- Confident with intramuscular injection administration
- Comfortable working within LPN scope under the direction of the prescriber and RN
- Steady, plain-language communication with patients who may be anxious, in withdrawal, or in crisis
- Successful completion of background check and drug screening prior to start date
- A pill-counting job. You will use your license.
- A place where nurses are treated as task-doers instead of clinicians. Your clinical read changes what happens next.
- A volume mill. We build panels and relationships, not throughput.
- Direct access to the prescriber. No message queue, no gatekeeping.
- Scheduling built around your life. Part-time contract hours set with you rather than handed to you — and you stay free to hold other engagements.
- Programs you help shape. SPRAVATO, IOP, and Virtual IOP are all growing — nurses here influence how they run.
- Care model worth working in. Trauma-informed, dual diagnosis, and a middle-of-the-road stance on substance use where a single lapse is met with clinical adjustment, not shame.
