Overview

Sub‑Acute, Long‑Term Care is an expanding sector of healthcare offering work‑life balance flexibility (no on‑call or weekend work). We are seeking a part‑time to full‑time (24‑40 hours per week) provider for our Grand Islander Center in Middletown, RI. This position is open to nurse practitioners and physician assistants, including new graduates.

The base salary range is $54 – $60 per hour, plus a no‑cap incentive compensation bonus program. Responsibilities

The Advanced Practice Provider reports directly to the Director of Clinical Practice and/or the Area Clinical Practice Manager, collaborating on‑site with the Center Leadership Team on site‑specific goals and priorities. The provider also collaborates with the Medical Director and/or Attending Physician for patient‑specific medical care.

Primary and urgent care services are delivered to nursing center residents alongside other medical providers. The provider works collaboratively with Center Leadership to achieve Align Med Partners’ mission of improving lives through quality healthcare and everyday compassion.

Qualifications
  • Graduate from an accredited School of Nursing program.
  • Licensed as a registered nurse.
  • Certified as a Nurse Practitioner (NP) by completing an accredited Nurse Practitioner Program with a Master’s in Nursing and a concentration in Older Adult and/or Family Nurse Practitioner (FNP). Other concentrations may be considered based on experience and business need.
  • Physician Assistant must graduate from a program accredited by ARC‑PA, pass the PA National Certifying Exam (PANCE) to use the PAC designation, hold PA state licensure, and have NCCPA certification.
  • A minimum of two years of experience working with adult or geriatric populations is preferred.
  • APRN with an unrestricted professional license and DEA in each state of practice.
  • PA with an unrestricted professional license and DEA in each state of practice.

Restrictions apply based on collective bargaining agreements, applicable state law, and factors such as pay classification, job grade, location, and length of service. Salary Range: USD $54.00 – $60.00 per hour.
