Licensed Practical Nurse (LPN) – Long Term Care in Middletown, Rhode Island, United States. This travel temporary assignment starts on 09/01/2026 and runs for 10 weeks. • Administer prescribed medications and treatments under the guidance of the supervising RN or physician.
• Monitor residents for changes in condition; perform routine assessments and document findings in the charting system.
• Assist with activities of daily living (ADLs), including hygiene, mobility, nutrition, and comfort measures.
• Support wound care, skin integrity, and pressure injury prevention per care plans.
• Collaborate with the interdisciplinary team to develop, implement, and update individualized care plans.
• Ensure safety, infection control, and patient rights are maintained; communicate changes to the care team promptly.

• Active LPN license in Rhode Island (or current eligibility to practice in RI)
• CPR certification (BLS) preferred
• Completion of an approved practical nursing program
• 1+ year of long-term care or skilled nursing facility experience preferred
• Ability to work a day shift (7:00 a.m. – 3:30 p.m.) and every other weekend; willingness to participate in a travel assignment
• Strong documentation and communication skills; competency with standard EHR systems
