Hospice Admission RN

We are a hospice agency serving the Tri-Cities area. The Hospice Admission RN will provide compassionate, emotionally supportive care to patients, ensuring dignity and comfort in their final days. This role includes oversight of team members, interdisciplinary collaboration, and continuous improvement of patient/family care plans.

• Assess patient’s physical, functional, psychosocial, and environmental needs.
• Implement and revise individualized plans of care.
• Ensure compliance with hospice policies, Medicare regulations, and quality standards.
• Consult and educate patients and families on disease processes, self-care, end-of-life care, and ethical issues.
• Supervise paraprofessionals providing services to patients.
• Prepare clinical and progress notes and participate in interdisciplinary group meetings.
• Participate in hospice performance-improvement programs.
• Maintain high level of customer satisfaction with clients, families, and caregivers.
• Communicate using current processes and technology within the organization.
• Conduct supervisory visits and provide feedback to team members.
• Attend field visits daily, weekly office meetings, and adapt to patient status on a full-time, on-call basis.

• Current Michigan RN license in good standing.
• Minimum 1 year of hospice experience.
• Empathetic approach to patients and caregivers.
• Ability to adapt to patient status and needs on a full-time, on-call basis.
