JOB PURPOSE:
The IDD Outpatient Biopsychosocial Intervention (OBI) Lead directs and trains staff responsible for OBI and Collaborative Care Case Management (CCCM). This position provides administrative program oversight and supports effective operation, quality assurance, and compliance of the OBI program. This role ensures that services are provided are in alignment with
HHSC standards, program requirements, and participant needs. This position supervises the IDD Collaborative Care Case Manager. This position works independently, under limitedsupervision, reporting major activities through periodic meetings. EDUCATION, EXPERIENCE, OTHER
QUALIFICATIONS:
Education Required:
ABachelors degree from an accredited college or university with a major in psychology, social work, medicine, nursing, rehabilitation, counseling,sociology, human growth and development, physician assistance, gerontology,special education, educational psychology, early childhood education or early childhood intervention or a bachelors degree with at least 30 hours of coursework in the previous fields. Experience
Required:
At least 2 years experience with individuals with intellectual and developmental disabilities or mental illness preferred. Registration, Certification, Licensure or other
Qualifications
Required:
Must maintain a valid Texas Drivers license, auto liability insurance and a driving record acceptable to Permia Cares insurance requirements. Required to pass criminal history and background checks as well as pre-employment drug screen. Must obtain QIDP and QMHP certification within 90 days of hire.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Ensure training and provision of CCCM, including, the Person-Centered Recovery Plan(PCRP) and Skills Training, using a person-centered approach focusing on theneeds and desires of persons served. Ensure
PCRP is led by the person served, embracing recovery, self-determination,community inclusion, respectful of the persons unique recovery journey. Assist with development and implementation of processes, procedures and checklists for referrals, intake process, biopsychosocial assessments, and other policies and procedures. Assist asneeded with Performance Contract Reporting requirements. Administer Bio-psychosocial Assessments for new admits. Provide education and training for community partners who may not understand how IDDand MH and/or IDD, MH and SUD manifest and how their unique ability to participate in the recovery process.
Support continuous improvement initiatives to improve CCCM and OBI program as a whole. Develop Skills Training to support goals discovered by person enhance outcomes across aarrange of quality of life areas. Collaborate with other LIDDAs to enhance continuity of services. Provide administrative and supervisory duties to ensure effective operation of the OBI program. Assist in resolving emergencies. Identify and resolve workplace problems. Meet unit performance measures or targets.
Maintain assigned caseload. Coordinate appropriate services to designated caseload. Enter accurate and appropriate documentation of services within timeframe required. Maintain confidentiality of sensitive records and treatment information, client filesand protected health information in compliance with HIPAA, laws, rules and regulations, and established procedures. Maintain regular and reliable physical on-site attendance. Regular attendance,dependability, and promptness are required for the scheduled work day 100% ofthe time, to ensure consistency and completeness of programs processes.
