As a Scheduler, you will play a crucial role in coordinating patient/client schedules, communicating with healthcare professionals, and ensuring the smooth operation of our home health care services. • Schedules and appropriately documents patient/client schedules in a timely manner
• Participates in coordinating care with management and the patient/client interdisciplinary team
• Provides effective communication to patients/clients, staff members, other healthcare professionals, and referral sources
• Responds to agency and patient/client needs in a professional and creative manner
• Demonstrates commitment, professional growth, and competency
• Promotes the Agency philosophy and administrative policies to ensure quality of care

• CNA or LVN preferred, but not necessary
• Minimum 1 year of experience
• Excellent interpersonal skills
• Valid driver's license and reliable transportation
