Hospital Discharge Coordinator

Permia Care is seeking a Hospital Discharge Coordinator to support safe, timely transitions from hospital to community care for behavioral health and IDD clients across West Texas. In this role, you will develop individualized discharge plans, coordinate follow-up services, and collaborate closely with hospitals, psychiatrists, nurses, social workers, and community providers. You will ensure accurate documentation, support client and family education, and help reduce readmissions while advancing Permia Care's mission to promote recovery, independence, and wellness in a compassionate, team-based environment.
