RN - Registered Nurse (PRN) - Home Health, Midland, TX
About Us:
Excela Care provides home health care focused on compassion, dignity, and patient-centered care. This role involves administering skilled nursing care in patients' homes and coordinating with a multidisciplinary team to support patients and families.
- Administer skilled nursing care for clients of all ages in their place of residence according to agency policy and clinical guidelines.
- Coordinate care with the multidisciplinary team, patient/family, and referring agency; assume responsibility for coordination of care; work independently without on-site supervision.
- Demonstrate the ability to transfer scientific knowledge from social, biological, and physical sciences to the nursing process.
- Administer medications and treatments according to agency policy and accepted standards of care.
- Communicate effectively with other nurses, patients, families, and physicians; document patient status, needs/problems, interventions, patient response, safety interventions, and education.
- Perform accurate assessments, develop and implement a written Plan of Care with physician orders, and adapt nursing interventions as patient needs change.
- Document patient status accurately, timely, and concisely; address patient needs, interventions, and education as per policy.
- Demonstrate organization and leadership by effective time and personnel management; supervise LVN/LPN and home health aide as appropriate.
- Maintain a cooperative attitude with staffing, promote teamwork, participate in case conferences and meetings; participate in on-call rotation as required.
- Perform related duties as assigned; maintain knowledge of Medicare/Medicaid and insurance standards to ensure reimbursement; comply with all agency policies and procedures; participate in continuing education and in-services.
- Maintain confidentiality of patient information and participate in risk management activities.
- Education:
Graduate of an accredited school of nursing. - Experience:
One year of nursing experience. - License/Certification:
Current State RN license; current CPR; current driver’s license and proof of auto liability insurance.
- Advanced written and verbal communication skills.
- Strong organizational skills.
- Ability to supervise in accordance with agency policies and applicable laws.
- Advanced math skills related to patient care.
- Ability to respond to inquiries or complaints from regulatory agencies or the business community.
- Cooperative attitude.
- Treat all information with confidentiality and security.
- Cooperate in risk management activities and investigations.
- Keep abreast of changes in health care law and ensure agency/program compliance with local, state, and federal laws and accreditation standards.
Physical Requirements
- Works in patients' homes in variable conditions; may involve exposure to blood and body fluids and infectious diseases.
- Ability to travel locally and work a flexible schedule; moderate noise and potential for stress.
- Prolonged standing/walking; ability to move up to 50 lbs and assist with patient handling; use of durable medical equipment in home settings.
