Job Summary

Licensed Practical Nurse (LPN) / Registered Nurse (RN) – Contingent 12‑hour shifts at Bristol Village, Waverly, Ohio. The facility is a 5‑Star rated skilled nursing, assisted living, and memory care community owned and operated by National Church Residences, the nation's largest provider of affordable senior housing and health care services.

• Provide nursing care on a 12‑hour shift schedule, supporting the daily needs of seniors in a skilled nursing, assisted living, and memory care environment.
• Work collaboratively with a multidisciplinary health care team to ensure quality care and patient safety.
• Document patient care activities, observations, and interventions accurately and timely.
• Participate in patient care planning, assessments, and care coordination meetings.
• Adhering to facility policies, state regulations, and best practice standards.

• Valid Ohio nursing licensure in good standing (LPN or RN).
• Graduated from an accredited nursing or practical nursing program.
• Experience in a skilled nursing, assisted living, or memory care setting preferred.
• Strong communication, team‑work, and problem‑solving skills.
