Job Title:
Travel Social Worker – LMSW / LCSW (Days)
Shift: Days – 5x8-hour shifts
Position SummaryUnder general supervision, the Social Worker provides comprehensive social services to patients and their families within an acute care hospital setting. This role
focuses on psychosocial assessment, discharge planning, crisis intervention, and coordination of care to support safe and effective transitions across the continuum of care.
LMSW or LCSW candidates only. Responsibilities- Provide social services to patients and families for assigned medical services.
- Conduct discharge planning evaluations to gather relevant clinical and social information.
- Perform psychosocial assessments and communicate findings to appropriate healthcare team members.
- Counsel patients and families regarding illness, treatment, emotional stress, social, and financial concerns.
- Identify and manage high-risk cases and provide crisis counseling related to abuse, neglect, and other critical situations.
- Deliver crisis intervention services for patients and families.
- Act as a liaison between patients, families, healthcare staff, and community agencies.
- Evaluate the effectiveness of services and recommend changes to care plans as needed.
- Coordinate referrals to community resources and ongoing support services.
- Assist with post-hospital placement and transfer to post-acute care facilities.
- Support patients and families coping with adverse reactions, illness adjustment, and changes in functional status.
- Utilize specialized interviewing and communication skills when working with children.
- Advocate for patients regarding informed decision-making, end-of-life care, ethical concerns, and goal setting.
- Prepare basic statistics related to referrals, extended stays, and service utilization as required.
- May provide supervision to students or newly hired staff.
- Perform other duties as assigned.
- Master’s Degree in Social Work from an accredited program.
- LMSW or LCSW required
- LMSW strongly preferred if not independently licensed
- Hospital or acute care experience preferred.
- Strong knowledge of social and psychological casework principles.
- Knowledge of hospital and community social service resources.
- Understanding of current social and economic trends affecting patient care.
- Strong analytical and problem-solving skills.
- Ability to clearly explain technical and clinical information to patients, families, and healthcare staff.
- Excellent interpersonal and communication skills.
