Job DescriptionCare Coordinator
OverviewThis position serves as a Behavioral Health Care Coordinator on the Central Office Care Coordination Team. The position works hand-in-hand with community provider agencies, community partners, and ODMHSAS program staff to ensure seamless collaboration and support. The primary goal is to coordinate care, offer outreach and engagement services, reduce hospitalizations, crisis stays, ER visits, and enhance overall access to care for these individuals.

- Bachelor's degree,
- Two years of professional experience,
