Job Title: LPN (Licensed Practical Nurse)

Location: Midwest City, OK

Department: Nursing

Reports to: DON

Position Type: 6am to 6pm, 6pm to 6am, PRN

Job Summary

As a Licensed Practical Nurse (LPN) at Emerald Care Center Midwest, you will be an integral part of our dedicated team of healthcare professionals committed to delivering exceptional care to our patients. Responsibilities
  • Provide direct patient care to residents, including administration of medications, wound care, and tracheostomy management
  • Conduct vital signs, monitor for changes in condition, and report observations to nursing staff
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team
  • Participate in care conferences, rounds, and quality improvement initiatives to ensure seamless coordination of care
  • Maintain accurate and detailed records of patient care and medication administration
  • Collaborate with other healthcare professionals to ensure patient needs are met, and positive outcomes are achieved
