St. Anthony’s Hospital in Midwest City is seeking a dedicated and compassionate Case Manager to join our System Case Management team. This weekend‑only position is ideal for professionals looking to make a meaningful impact while enjoying a consistent and focused schedule.

Location: St. Anthony’s Hospital – Midwest City, 2825 Parkland Dr., Midwest City, OK 73110

Schedule: 8:00 a.m. – 2:00 p.m. (any four days between Monday and Friday)

Hours:

24 hours per week

• Coordinate patient care and discharge planning.
• Collaborate with interdisciplinary teams to ensure quality outcomes.
• Advocate for patient needs and connect them with appropriate resources.
• Maintain accurate documentation and ensure compliance with hospital policies.
• Ensure compliance with Condition of Participation (CoP) discharge planning guidelines.
• Identify and address gaps or barriers related to inpatient and observation length of stay.
• Assess patients at high risk for readmission and develop plans to reduce readmission risk.
• Participate in process improvement initiatives and standard work efforts.
• Support utilization management, ED case management, admission processes, and act as liaison with post‑acute providers such as skilled nursing facilities, rehabilitation centers, and home health agencies.
• Apply evidence‑based practice and scientific knowledge to nursing care, ensuring care is tailored to each patient’s age‑specific and clinical needs.
• Demonstrate professional nursing standards and adhere to the ANA Code of Ethics for Nurses in all patient interactions.
• Maintain constant state of alertness and safely perform duties.
• Perform other duties as assigned by the Manager.

• Registered Nurse (RN) licensure in the state of employment (Illinois, Missouri, Oklahoma, or Wisconsin).
• Three years’ acute‑care experience.
• RN or relevant licensure/certification in case management or social work.
• Strong communication and organizational skills.
• Ability to work independently during weekend shifts.

• Registered Professional Nurse (RN) – Illinois Department of Financial and Professional Regulation.
• Registered Nurse (RN) issued by Compact State or based in Missouri, Oklahoma, or Wisconsin as authorized by the respective state Board of Nursing.
