Charles

F. Hayden School is a special education clinical school committed to serving children grades K through 12. Youth are referred to out of district placement by their LEA in order to receive services in a clinical therapeutic milieu setting. Educational plans follow each student’s IEP and transitional/vocational services are offered to youth as part of their educational plan.

Position Overview

The Transitional Coordinator oversees the Job Coach and the vocational opportunities for youth at Charles F Hayden School by coordinating opportunities in the school setting and local community. The Transitional Coordinator is responsible for assessing students’ readiness for services, teaching prevocational skills and supervising students while in placements. The coordinator tracks and presents progress data to school staff, LEAs, and families.

Essential Functions and Responsibilities
  • The coordinator assists in overseeing Charles

    F. Hayden Vocational/transitional Program by organizing vocational opportunities in the school and local community and completing all transitional requirements needed for the clients.
  • Supervises Job Coaches and develops key relations with the school and other agency staff to promote Transitional/Vocational Services.
  • Works with interdisciplinary school team to assess transitional needs of identified students as outlined in student’s IEP goals.
  • Assist school teams in determining appropriate transitional assessments (formal and informal) to conduct based on student profile (e.g., Oasis, LIST).
  • Implement vocational rubrics and data progress monitoring systems to assess student progress in regard to their transitional goals and present to teachers for PPTs.
  • Coordinate with school team to prepare transition plans and monitor data systems (vocational observation form) to assess student progress, debriefing with classrooms and SWs.
  • Collaborate with on-site vocational supervisors to set goals and receive feedback.
  • Provides supervision, job coaching and individual performance feedback to students while they are working in their placement.
  • Collaborates with social workers to ensure individualized safety plans are followed during transitional placement.
  • Meets weekly for supervision with supervisor.
  • Follows the BGV model and TCI to ensure safety of all Charles

    F. Hayden youth.
  • Other duties may be assigned by the VP of Ed and to serve BGV needs.
Essential Qualifications and Skills
  • Requires a minimum of a bachelor’s degree in human/social service Field.
  • Minimum of 1 year’s experience working with students within transitional programming within programming and community settings.
  • Minimum of 1 year’s experience working with secondary level students with social/behavioral challenges.
  • Demonstrate proficiency in writing and reporting on transitional progress.
  • Must fulfill competency‑based training requirements in TCI and Boys & Girls Village Social Skills Model.
  • Must be able to respond to crisis, including the ability to physically restrain if necessary.
Preferred Qualifications
  • Previous management experience preferred.
  • Experience with data collection and analysis regarding furthering transitional programming.
  • Commitment to learning as an ongoing opportunity and professional imperative.
Supervisory Responsibilities
  • Supervise job coaches.
Physical Demands
  • Must be able to respond to crisis, including the ability to physically restrain if necessary.
Environment
  • Must be willing to work a flexible schedule.
  • Valid driver’s license and clean driving record.
  • Must be willing to drive to students off site and attend meetings in the community.
  • Some evenings and weekends may be required.
